http://icopy.sourceforge.net/ - I've used this and it's excellent. Disable the preview to skip the GUI that comes from your scanner. Make sure to use 1.6.1 or newer, I had scanner page size issues in 1.6.

A few programs to use a scanner as a photocopier:
I have yet to find one that works as well as the "Copy" program that was included with my first scanner, a Umax. Unfortunately, the program that came with my new multi-function Lexmark doesn't let you choose the printer, or even the resolution. Yuck.
Anyway, these are OK. They all have free trials or free versions:

The built in document scanners of some MFC printer/fax/scanners is terrible... I'd like a cheap or free alternative - I want PDFs that do not consist of giant uncompressed bitmaps!

TWAIN vs WIA - TWAIN uses device manufacturer UI, WIA uses a generic UI. Thus, TWAIN, when working, can have better controls.
