Simple Way to Create an Icon

A simple way to create an icon (such as favicon.ico for your website!).

Grab a copy of irfanview (it's free for personal use).
Create (or resize an existing graphic) to 16 pixels by 16 pixels.
Open the file in irfanview and do a "save as" to a .ico.
You're done.
Since irfanview will open a whole lot of graphics types, you have quite a lot of flexibility. But you are probably best using a .bmp rather than a .jpg or .gif because you don't have compression artifacts to worry about.
